Submit Headshot Photo
You must submit a headshot photo (originals, not copies) of yourself. Your headshot should be a passport-like photo. When ABCO develops its on-line directory of certified doctors, this photo will be used, so it should present you in the light you'd want the public to see you.
If submitting your application form electronically via Email, if you have a JPG headshot photo of yourself, you can attach that JPG photo to your email when submitting your form. If submitting your application via regular mail please include two passport sized headshot photographs with your application (be sure to write your name on the back of each photo).
